We are seeking an experienced Registered Nurse to join our team!

Care Coordinator Job Description includes but is not limited to:

Follow established standards of nursing practice and impliment facility polices and procedures

Supervise and evaluate all direct resident care staff and initate corrective action if necessary

Conduct resident rounds daily, reporting any concerns to DON

Attend staff meetings , care conferances and in service meetings

Perform and document comprehensive assessments of each resident

Assist with Resident Admissions and Discharges

Complete Quaterly / Significant Change Assessments

Create and Update Resident Care Plans Daily

Update Remote MDS nurse daily on any changes

Update PASRR Reivew as needed

Rotate Manager On call Duties

Work with Activity Director to establish activities which will promote exercise and mobility

Supervise Restorative Aide in ROM and complete charting
